Update your student’s information with EBF
It’s that time of year again! As your children are packing up
to return to college, make sure that their CSEA Employee Benefit Fund (EBF) benefits are up-to-date before they go.
The EBF requires dependents over age 19, up to age 25, to provide proof of student status annually for benefits to continue.
If your child is a full-time college student, he or she is still eligible to receive coverage through EBF.
There are several ways you can provide proof of student status:
You can submit student proof quickly and easily online by visiting the EBF website cseaebf.com. Once there, click the “Enroll Online” button, enter your
Social Security number and click on “submit student certification.” Complete the form and submit.
If you would like to print a paper copy of the form so you can fax
or mail it in, click the “Download Forms” button on the EBF website home page, then select the Student Proof Form for the applicable year.

If you do not have access to a computer, please feel free to call the EBF directly at (800) 323-2732 and we will be happy to mail you the form.

As this edition went to press, an agreement had not yet been reached between Empire Blue Cross/Blue Shield (the hospital administrator for the Empire Plan) and South Nassau Communities Hospital.
If an agreement is not reached by midnight on June 30, 2018, South Nassau Communities Hospital will be considered
an out-of-network provider with Empire Blue Cross/Blue Shield effective July 1, 2018.
After June 30, 2018,
there are only certain
specific circumstances in
which Empire Plan enrollees
may be able to be approved for continued services through South Nassau on an in-network basis by Empire BlueCross BlueShield;
• Cases of emergency;
• If no in-network hospital exists
that can provide the services
required;
• If a network hospital is not
available within a 30-mile
radius from your home;
• For continuation of care for
pregnancy or health risk; • For any services that were
previously authorized.
If enrollees choose to use South Nassau, all other services will
be covered on an out-of-network basis. Empire Plan enrollees will
be responsible for paying 10 percent of billed charges for
covered services, up to the annual coinsurance
maximum amount. It is in the best
interest of Empire Plan enrollees to seek health
care at hospitals that participate with Empire
Blue Cross/Blue Shield. Below is a listing of alternative hospitals in Long Island and Queens
that continue to participate with Empire Blue Cross/Blue Shield.
Enrollees who are currently receiving services from South Nassau or are scheduled for treatment at South Nassau should call Empire Blue Cross/Blue Shield toll-free at 1-800-495-9323, Monday through Friday, between 7 a.m. and 8 p.m.
