Danny Donohue and the EBF Board of Trustees recently approved a new EBF Retiree Vision Plan.
The plan, which will take effect
for those members who retire on or after June 1, 2016, offers a flexible and smart way for retirees to save on vision care costs.
“We are always trying to find better ways to serve our members and provide benefit options to active members and our retirees,” Donohue said. “This new Retiree Vision Plan
is just one more way that we are demonstrating the value of your union membership.”
The EBF Retiree Vision Plan offers eye care services with no out-of- pocket costs when the member or eligible dependent uses the plan with a participating Davis/Visionworks provider. To maximize the value of the new plan and make it useful to retirees located all over the United States, the EBF’s provider panel now includes a national network.
one pair of glasses (lenses and frames) or initial supply of contact lenses once every 12 months from the date of the last appointment. The plan includes: plastic, polycarbonate, glass, standard or premium progressive no-line bifocals or trifocals, glass photochromic lenses,
cataract lenses, fashion tints, prescription sunglasses
and scratch guard coating.
Plan contact lenses
consist of soft planned replacement or disposable types. For non-plan contact lenses, the plan provides $125 toward the cost. Fixed co-payments are also available thru participating providers for certain items not covered by the plan. These are only available during
provider's office whenever medically necessary.
Who is eligible?
You must be a CSEA bargaining unit member
retiring on or after June 1, 2016.
In addition, you must
have
been a participant of a
negotiated vision program as an
active employee on your last day of employment. CSEA members who are retiring have a maximum of 90 days from your last day of employment to elect to join the new Retiree Vision Plan.
• New York State employees
in bargaining units ASU, ISU, OSU, or DMNA will be eligible on June 1, 2016.
CSEA members covered by
the EBF who retire will receive information (if eligible) on the EBF’s Retiree Dental and Vision benefits in their COBRA election notice.
