Here at the CSEA Employee Benefit Fund (EBF), we are proud
of the benefits CSEA members have negotiated into their contracts. It turns out a few of our favorite things are yours, too:
Cleanings and Exams
All of EBF’s dental plans offer three cleanings and three exams
per person per year. These services will not be deducted from your
plan’s annual maximum. Along
with brushing your teeth twice a
day with fluoride toothpaste, daily flossing, and a healthy diet that limits sugary beverages
and snacks, seeing
your dentist for
regular checkups
and cleanings is an
important part of
dental care at home.

Network Providers
Participating providers agree
to accept EBF payments as full payment so there are no out-of- pocket expenses for most covered services. Search for a provider online at cseaebf.com. 70% of our members use a dental provider, and almost all
members use a participating vision provider.
Split Vision Benefit
Use a participating provider to have your exam and select your eyewear on the same day or use your exam benefit and return to the same provider later in the calendar year to
select your eyewear. Or, use a participating provider for your exam and select a different participating provider to get your eyewear during the calendar year.
Always refer to your summary plan descriptions for
detailed information about your plan including limitations and exclusions. If you are unsure which benefits
have been negotiated for your bargaining unit, use our Benefit Search tool at cseaebf.com. Select “Local Government” or “State Agency.” If you select “Local
Government,” select the county of your employer, click search and then select your unit. If you select “State Agency,” you will need to select your specific agency from the menu.

***Clarification for April article:
For employees with our State Vision plan, dependents must be under age 19 to qualify for a vision exam and materials every year. All dental plans, except NYS Retiree, include orthodontic coverage. Appliances must be in place before age 19. Orthodontic coverage is provided for employees under age 19 and unmarried dependent children enrolled in the plan. Four of our plans, UCS, UCS Retiree, Equinox and Dutchess, also include an adult orthodontic benefit provided for employees and their spouses.***
