Make it easier for EBF to help: a guide to maintaining your records It is important to keep track of all the places you need to contact when an important life event occurs. The CSEA Employee Benefit Fund (EBF) is an important part of that planning. Here is a quick guide to the EBF issues most frequently encountered: Dependent status Coverage offered by the EBF is not automatic. You must complete an enrollment form that will be mailed to you in a “Welcome Aboard Packet” after your employer has notified us that you are eligible for benefits. Once you and your dependents are fully enrolled, it is the member’s responsibility to inform the EBF of any changes. Notifying EBF of changes in full-time student status The EBF requires that current proof of student status be provided annually for dependent children under age 25 who are attending college or an accredited institution full time for coverage to continue. The member must notify the EBF immediately if your dependent(s) no longer meets full time student status. Failure to notify the EBF of accurate student status could result in delay of claims processing or result in claims collections against the member. Notifying EBF of legal separation It is your responsibility to notify the EBF of legal separation or divorce from a spouse. Members must submit to EBF a “Remove Dependent” form with a copy of the separation/ divorce papers or a letter from an attorney indicating that you are legally separated or divorced. Change of address To update your address, visit EBF’s website at cseaebf.com and click on the “Change of Address” icon on the home page. Obtain forms Visit cseaebf.com to get all EBF forms. From the home page, select the “Download Forms” button. Available forms include the Enrollment Form, Student Proof, Remove Dependent and all miscellaneous Co-Pay Reimbursement Forms.
