Sudden vision changes — EBF has you covered
The CSEA Employee Benefit Fund (EBF) provides vision coverage for more than 105,000 CSEA-represented New York state employees and their dependents.
Plan documents providing detailed information about the EBF benefits available to New York state bargaining unit members

The benefit coverage offered to members is comprehensive.
Recently, some questions were asked about what happens when a vision prescription changes due to a medical situation. Good news — EBF has you covered! With the proper medical documentation, your network vision provider can access a special plan rider that allows a new pair of eyeglasses or contacts because of a change in your vision health.
What happens if my vision prescription changes because of a medical condition?
New York State Vision Plan members enrolled in the EBF
have access to a special plan
rider. Such members are eligible for an additional exam and a new pair of eyeglasses (or contacts) with the new prescription, when deemed medically necessary by Davis Vision. This provision is known as “The Additional Services Program.”
The program is available for those who use “In Network”
providers. Members must
provide documentation from
their medical professional
stating they are receiving care
for one of the qualifying medical conditions under the Medical Exception program to a Davis Vision provider. The Davis Vision provider then submits a prior approval form to Davis Vision with documentation attached to the request for services.
Under the policy, the qualifying conditions are:
• Patient is Diabetic
• Patient has Glaucoma
• Patient has Cataracts
• Patient has Keratoconus.
• Patient has had eye surgery
within two years of their
last prescription. • Patient is taking
prescription drugs whose side effects cause vision changes.
• Patient has other documented medical conditions which could reasonably be expected to result in a change of refractive status.
