2015 New York State Health Insurance plan rates for CSEA active state employees and Unified Court System employees Continued from page 16 New York state employees who currently participate in the Opt-Out Program will receive a notice from Civil Service advising that the PS- 404 and PS-409 forms must be resubmitted during the Annual Option Transfer Period in order to continue participation in 2015. Employees interested in participating in the Opt- Out Program for 2015 should see their agency health benefits administrator during the Annual Option Transfer Period for additional eligibility criteria, plan parameters and required paperwork no later than Jan. 16, 2015. New York state pays the majority of the premium cost for coverage regardless of the plan an enrollee chooses. When considering health insurance options for 2015, please consider all anticipated costs throughout the year, not just the biweekly paycheck deductions. Keep in mind the out-of-pocket expenses an enrollee is likely to incur during the year, such as co-payments for prescriptions and other services, coinsurance and deductibles if using providers not covered under the plan. Although the Open Enrollment period has been extended to Jan.16, 2015, the start date of new health insurance options will occur with the payroll period closest to Jan. 1, 2015. On the chart is the 2015 biweekly rates for CSEA New York state active employees. Please note there are two sets of rates. The first set reflects 2015 biweekly rates for CSEA represented state employees who are Grade 9 and below. The second set reflects 2015 biweekly rates for CSEA represented state employees who are Grade 10 and above. Enrollees seeking additional information can contact their agency health benefits administrator for additional information prior to the Jan. 16, 2015 deadline or contact the Joint Committee on Health Benefits within the CSEA Health Benefits Department at 1-800- 286-5242. CSEA membership/agency shop fees effective Jan. 1, 2015, for biweekly payrolls The scheduled adjustment in membership dues and agency shop fees is effective Jan. 1, 2015, to reflect AFSCME’s minimum dues structure approved by delegates and the AFSCME convention. The change applies to annualized salaries, not including overtime or location pay, based on salary at Jan. 1, 2015.
