Don’t miss the reimbursement deadlines! The CSEA Employee Benefit Fund offers different prescription drug and/or physician co-payment reimbursements that may have been negotiated into your collective bargaining agreement. These benefits reimburse you once annually for co-payments spent per calendar year and if you’ve already reached the plan limit, you can submit now! The deadline for the 2015 calendar year is March 31, 2016. Forms can be obtained by visiting our website at cseaebf.com under the “Download Forms” section. If you are eligible, please submit the applicable claim form along with supporting documentation by March 31, 2016. For prescription drug co-pay reimbursements, attach an itemized pharmacy printout. For physician co-pay reimbursements, attach Explanation of Benefits (EOBs) from your health insurance carrier. If you have questions about these benefits please contact our Member Services Department at 800-323-2732. Below is an overview of the plans: Prescription drug co-pay reimbursement for state employees Members who are enrolled in the New York State Health Insurance Program (either the Empire Plan or a health maintenance organization) are entitled to reimbursement once annually for NYSHIP prescription drug co-pays and covered prescriptions less than the co-pay for themselves and their eligible dependents. Once co-pays reach $300, the next $150 in prescription drug co-pays is reimbursable. If you still have not accumulated $450 by Dec. 31, 2015, submit your claim for what you did pay over $300. The deadline for the 2015 calendar year is March 31, 2016. Prescription drug co-pay reimbursement for state Liquidation Bureau employees Members who are enrolled in the New York State Health Insurance Program (either the Empire Plan or a health maintenance organization) are entitled to reimbursement once annually for NYSHIP prescription drug co-pays and covered prescriptions less than the co-pay for themselves and their eligible dependents. Once co-pays reach $300, the next $100 in prescription drug co-pays is reimbursable. If you did not accumulate $400 by Dec. 31, 2015, submit your claim for what you did pay over $300. The deadline for the 2015 calendar year is March 31, 2016. Unified Court System combined co-pay benefit Reimburses prescription drug copays and co-pays for physician office visits. The claim can be made up of both types of co-pays for a combined maximum reimbursement of $325. The deadline for the 2015 calendar year is March 31, 2016. Unified Court System retiree co-pay Reimburses prescription drug co-pays only up to a maximum of $100. Physician co-pays for office visits are reimbursed up to a maximum of $125. The deadline for the 2015 calendar year is March 31, 2016. Local government co-pay For local government employees who have a negotiated prescription drug co-pay or physician co-pay reimbursement benefit: Submit the claim along with your itemized pharmacy and/or Explanation of Benefits (EOB) from your health insurance carrier. Please contact either our Member Services Department or refer to your collective bargaining agreement to verify the level and type of reimbursement available for you. The deadline for the 2015 calendar year is March 31, 2016. Open enrollment period for 2016 flex spending accounts run through Nov. 9, 2015 The Health Care Spending Account (HCSAccount) allows you to set aside up to $4,000 in pre-tax salary to pay for health-related expenses not reimbursed by your health insurance). Please note that recent changes to federal law have affected some of the rules under which the HCSAccount operates. Before you enroll, carefully review the information on the Flex Spending Account website concerning restrictions on over-the-counter drug reimbursement, as this may affect the amount of money you wish to contribute to your account. The Dependent Care Advantage Account (DCAAccount) allows you to set aside up to $5,000 in pre-tax salary for eligible child care, elder care or disabled dependent care expenses). If you are currently enrolled in one of the flexible spending accounts, you must re-enroll to continue your participation in 2016. NYSHIP option transfer period coming soon Dates to be determined Benefits eligible employees will have the ability to change their health insurance plan for the 2016 plan year.
